Abstract
This paper presented the results of evaluation of one of the community health center based health promotion program for about two years according to Solomon¡¯s four group design measured by modified health promotion outcome model. The range of evaluation indicators were health promotion actions, health promotion outcomes, intermediate health outcome and, health and social outcome indicators by the effectiveness and sensitivity of the responses to intervention. The evaluation findings showed that, the intervention had unnotable impact on the smoking rate, but had positive impact on the stage of change of smoking cessation, and had positive impact on the stage of change of exercise in female. In summative evaluation, the result of community health center based health promotion programs during short periods showed that very positive results in short term impact evaluation, but mixed and vague results in intermediate and long-term outcome evaluation. But we would like expect to the positive improved results after long term intervention.
